The successful applicants will be required to work with an allocated caseload notably older people and younger adults with disabilities in a hospital setting, working collaboratively with health colleagues and other partner agencies. These posts will help us continue our innovative work to put people at the centre of care and support in Lincolnshire. Creativity and problem solving will be crucial to be able to respond to people who may at times be in crisis. A person centred approach when assessing and support planning is required along with a commitment to multi professional engagement. We aim to improve quality of life, promote and enable independence by using a variety of interventions and multi- disciplinary working Under the Care Act 2014. The person must show ability to work autonomously in identifying and addressing need and risk, and have the ability to assess the quality of service offered to an individual and to act to ensure a good quality of provision.  Be able to promote the social inclusion of adults and families you work with.                                       

The post holders will work as part of the 7 day Adult Care rota on-site at Pilgrim Hospital; supported by a Hospital Manager. This equates to approximately 1:4 weekends worked, and these are all scheduled 3 months in advance.

 Newly qualified social workers will have the opportunity to join the ASYE programme as part of their on-going development.

The social worker post holder must hold a professional social work qualification e.g. DIPSW, CQSW, CETSW, CSS or BA Social Work and ensure that registration with the HCPC is maintained and renewed appropriately and is able to demonstrable commitment to continuous professional development and to work as part of a team, learning from and contributing to the development of others. It is essential that the social worker is able to show sound knowledge of relevant legislation underpinning statutory Adult Care Practice; be able to demonstrate values that promote wellbeing, challenge oppression and discriminations and empower people to live well. There is an element of organisational and administrative skills necessary for managing a caseload.
