Qualified Practitioner Social Care - Maternity Cover

We are seeking a Qualified Practitioner at level 1 or 2 with an enthusiastic and positive 'can do' approach to join our Intake and Transitions Team on a permanent, full time basis. About the Role

The Intake and Transitions Team operates county wide within the Learning Disability Service and their main priority is to support effective transition for eligible young people moving from Children's to Adult Services.

The team’s priority is supporting young people to make informed choices about their future and assisting them to achieve the best outcomes during transition and as they enter adulthood.

Working in partnership with Children's Services and relevant care and health partners, each staff member has a link worker role with relevant local schools; attending Education health and care (EHC) reviews and providing information and support to young people, their families/carers and to our colleagues in educational establishments.

About the Team

The team is a small, friendly and supportive. Consisting of Qualified Practitioners (Social Workers and Learning Disability Nurses) and Unqualified Practitioners, who work together to achieve the best outcomes for the young people they support.

Our diverse and welcoming team support each other daily, meet collectively on a weekly basis to catch up about workloads, have a chat and catch up with each other. Our monthly get togethers are face to face and allow us brush up on best practice, caseloads, information sharing and give feedback around what is working well for us.

About you

You will need to hold a Nursing or Social Work qualification (DIPSW, CQSW, CETSW, CSS or BA/Bsc / Social Work). You must also hold a current registration with the NMC or Social Work England. 

You should be able to demonstrate commitment to continuous professional development and to work as part of a team, learning from and contributing to the development of others.

It goes without saying that it is essential that you can show sound knowledge of relevant legislation underpinning statutory Adult Care Practice, and be able to demonstrate values that promote wellbeing, challenge oppression and discriminations and empower people to live well.
