ESCO Keyworker
Lincolnshire County Council's Early Support Care Coordination service (ESCO) is recruiting to three ESCO Keyworker positions primarily based in East Lindsey, South Kesteven and Lincoln.
About the Role
ESCO is a service which focuses on supporting services and practitioners to work in partnership with children, young people and their families to overcome barriers and work towards positive outcomes.
Our aim is to improve access to and delivery of services for families who have children with significant or profound special educational needs and disabilities.
ESCO enables services to coordinate their activity effectively and provide families with a single point of contact and continuity through care coordination, information sharing and signposting.
ESCO Keyworkers provide coordination of services through regular review meetings and drawing up a robust Family Support Plan that identifies any worries and how best to address them.
About You
Knowledge and skills required (not exhaustive)
- You will need to hold a Health, Education or Social Care qualification such as NVQ 3, NNEB
- Ability to communicate effectively with children, parents, colleagues and other agencies
- Knowledge of legislation relating to disabled children
- Knowledge of SEND processes
- Ability to chair complex care meetings and ensure fair and equitable participation of all participants
