Community Care Officer Level One or Two
An exciting opportunity has arisen to join the Skegness/Boston Learning Disability Care Management Team. The position is a 18.5 hour permanent Community Care Officer Level one or Level two post and will be based in Skegness.
We are looking for an enthusiastic and motivated person to join our friendly and busy care management team. The successful candidate will need to have a positive and 'can do' approach to their work.
You will need to demonstrate good communication, literacy and numeracy skills and must have the ability to keep well written case notes. You will be required to follow the policies and procedures in place and will need good IT skills which will include being able to follow our IT systems processes effectively and within good timescale.
You must be able to work well independently, as well as part of a team including with service users, their families and other agencies and our health colleagues.
You will be required to carry out strengths based assessments in line with the Care Act 2014, complete continuing health care checklists and arrange cost effective packages of care for people who are eligible for funded services.
You must be able to travel effectively and within reasonable timescales to meet the demands of the service.
This post is subject to 7 day working.
We anticipate interviews will take place on 18th June 2019.
