There is currently a Part time, Permanent position based in the East of Lincolnshire for a Community Care Officer Level 2 (£25,295 - £27,905 per annum pro rata) 20.5 hours a week, Based in Skegness

 

A challenging but exciting time for Lincolnshire Adult Care Services with the implementation of the Care Act 2014 and requirement for all care services to work in a more integrated way, is presenting many opportunities.

The Adult Care team provide support for older adults (65+) and people with a physical disability and their carers. Promoting people's independence, recognising and celebrating the person's strengths, utilising community resources and social networks to enable the person to live a full, rewarding life, is the key objective.

The successful candidates will be creative and passionate about supporting people in a person centred way. They will be able to work in local and multi-disciplinary teams and will take responsibility for an allocated caseload under the direction of a line manager/supervisor. The ability to work on their own initiative and to reflect on their practice, ensuring continuous professional development, along with good verbal and written communication skills is essential.

Applications are invited from people who have worked in social care and can demonstrate interest +/or experience in areas such as;

Strength based Adult Care Assessments, Care Planning, Continuing Health Care funding, Direct Payments, Mental Capacity Act Assessments and Best Interest decision making. Planning Transitional Care & Reablement opportunities along with ability to identify equipment and adaptations to include Disabled Facility Grants would be advantageous.
