Community Care Officer Level 2

We are currently looking to recruit 4 new Level 2 Community Care Officers into our Adult Care team on the east coast.
We have the following contracts available;
- 1 x permanent - full time (37 hours)
- 1 x permanent - part time (20.5 hours)
- 2 x fixed term/secondment (18 months) - full time (37 hours)
All positions are based out of Skegness however travel will be required (currently working remotely where possible).
To be successful in these roles candidates must have a minimum of two years' experience working in a similar health or social care position. They will be creative and passionate about supporting people in a person centred way. Be able to work in local and multi-disciplinary teams and take responsibility for an allocated caseload under the direction of a line manager/supervisor. The ability to work on their own initiative and to reflect on their practice, ensuring continuous professional development, along with good verbal and written communication skills is essential.
Applications are invited from people who have worked in social care and can demonstrate experience in areas such as;
- Valuing people using strength based models to assess and plan support with people.
- Identify eligibility for Continuing Health Care funding and supporting people through the CHC process.
- Seeing Direct Payments as a way of offering people more choice and control in their lives.
- Understanding the principals of the Mental Capacity Act and Best Interest decision making.
- Working creatively, using technology to assess, plan, implement and evaluate people's aspiration and goals.
Supervision and training will be offered to the successful candidates to improve their current knowledge, skill and confidence.
The Adult Care team provide support for older adults (65+) and people with a physical disability and their carers. Promoting people's independence, recognising and celebrating the person's strengths, utilising community resources and social networks to enable the person to live a full, rewarding life, is the key objective.
The team consist of Community Care Officers, Social Workers and a Lead Practitioner.
They are based at the Aura Business Centre on Heath Road Skegness but as agile workers are able to work from home.
Completing Adult Care Assessments and planning Care & Support for vulnerable people in the Skegness area requires practitioners to be able to travel to people's homes, care homes and sometimes hospital settings.
